the results of this study and the knowledge gained from more than a decade of randomized trials of non-oxynol-9 as a vaginal microbicide, it is time to move on. Women at risk for STIs such as chlamydia and gonorrhea are often also at risk for HIV-1. In practical terms, a vaginal microbicide must have no risk of increasing HIV-1 incidence before it can be introduced as a microbicide for other STIs. Given its poor safety profile in frequent users, non-oxynol-9 is clearly not that product. Despite more than 15 years of urgent appeals for an effective vaginal microbicide for prevention of HIV-1 and other STIs, few clinical trials of microbicides have been published. In contrast , several studies of mother-to-child transmission of HIV-1 have published results that have had a dramatic effect on reducing perinatal HIV-1 transmission rates. Whatever the reasons for this difference, during the past 5 years, while research on vaginal microbicides has inched forward, more than 10 million women worldwide have acquired HIV-1, the vast majority of whom live in the developing world. 17 Although the message that nonoxynol-9 is not an effective vaginal microbicide is disappointing, it should not discourage further microbicide research. Clearly, there remains a need for an inexpensive, effective, female-controlled method for preventing STIs, and there may be many studies with negative results before one is found. Research on other vaginal microbicides, conducted by individual investigators as well as by research networks, should be encouraged. 18 The study by Roddy et al can serve as an example of a well-designed and executed trial that has moved the field of microbicide research another step forward. Association between cannabis use and psychosis-related outcomes using sibling pair analysis in a cohort of young adults.
CONTEXT Prospective cohort studies have identified an association between cannabis use and later psychosis-related outcomes, but concerns remain about unmeasured confounding variables. The use of sibling pair analysis reduces the influence of unmeasured residual confounding. OBJECTIVE To explore the association between cannabis use and psychosis-related outcomes. DESIGN A sibling pair analy...
متن کاملStronger evidence is needed before accepting that cannabis plays an important role in the aetiology of schizophrenia in the population
Schizophrenia is a debilitating but poorly understood condition with very few known modifiable risk factors. Cannabis use can acutely induce psychotic experiences, but its causal relationship to schizophrenia is less well understood. Longitudinal cohort studies suggest that the association between cannabis and psychotic outcomes is not due to chance or reverse causation. However, the associatio...
متن کاملAn evidence based review of acute and long-term effects of cannabis use on executive cognitive functions.
Cannabis use has been shown to impair cognitive functions on a number of levels-from basic motor coordination to more complex executive function tasks, such as the ability to plan, organize, solve problems, make decisions, remember, and control emotions and behavior. These deficits differ in severity depending on the quantity, recency, age of onset and duration of marijuana use. Understanding h...

متن کاملPreliminary evidence for a sex-specific relationship between amount of cannabis use and neurocognitive performance in young adult cannabis users.
Accumulating evidence suggests neuropsychological deficits from cannabis use, with a burgeoning area of preclinical research indicating possible sex-differences. However, few studies have examined how cannabis use may differentially impact neurocognition in male and female cannabis users. As such, we examined potential sex-differences in associations between amount of cannabis use (across sever...
متن کاملCannabis effects on driving skills.
BACKGROUND Cannabis is the most prevalent illicit drug identified in impaired drivers. The effects of cannabis on driving continue to be debated, making prosecution and legislation difficult. Historically, delays in sample collection, evaluating the inactive Δ(9)-tetrahydrocannabinol (THC) metabolite 11-nor-9-carboxy-THC, and polydrug use have complicated epidemiologic evaluations of driver imp...
